Welcome to the NCLEX-PN practice test on Physiological Adaptation, one of the most important subcategories of Physiological Integrity. It focuses on providing care for clients with acute, chronic, or life-threatening physical health conditions. There will be 7 to 13% of all the examination questions about this subcategory, not as many as others, but it will evaluate how well you know pathophysiology. Physical Adaptation covers alterations in all body systems, so you should be familiar with conditions that require nursing care for each, such as tracheostomies, dialysis, or seizures. Also included are complications of pregnancy, labor and delivery. Wound care, fluid and electrolyte imbalances, medical emergencies, and unexpected responses to prescribed therapies. This practice test covers pressure ulcers, chronic disorders, acute and trauma situations, client education, and health promotion. Start this test whenever you’re ready. If you leave any test before it’s completed, it will remind you where you left off when you return. Read each question carefully and try to think of the answer before looking at the options. Remember to select the BEST choice. If you’re not sure, click on the Hint button under the fourth option to get a clue. When you select the correct response, you’ll go the next question. If you make a mistake, an explanation will immediately pop up. Each time you take a practice test, the questions and options are shuffled into a different random order. Both of these techniques have been proven to reinforce your learning.
